Initiating a repair or replacement for PPE follows a formal request workflow that captures all necessary details and routes the request through the proper channels. The SRS requisition form is designed for these maintenance and repair needs. It records exactly what item is involved (the brush helmet), the issue or replacement requirement, asset identifiers, location, urgency, and requester information, and it moves the request into the department’s supply/maintenance system for authorization, parts provisioning, and scheduling. This creates a traceable record and ensures the repair or replacement is handled through the correct process, with accountability and proper budgeting.

Work order forms are typically used to document tasks for a maintenance crew after a request has been approved, rather than to initiate the repair or replacement itself.
